Phoenix Health Center of Family Health Centers

Family Health Centers, Inc., is a not-for-profit community health center that provides a variety of health care services regardless of a person’s health insurance status or ability to pay for services. They provide services to more than 37,000 patients annually through seven Family Health Centers sites throughout Louisville Metro.

Since 1988, Family Health Centers has operated a Health Care for the Homeless Clinic in the Louisville Metro region.  Centrally located near downtown Louisville and close to the city’s homeless shelters, FHC- Phoenix services more than 6,000 homeless adults and children each year.

FHC-Phoenix provides the following services:

  • Primary Health Care
  • Tuberculosis Testing
  • Dental Services
  • Laboratory
  • Social Service Outreach, Assessments, and Case Management
  • Mental Health Outreach Services
  • Mental Health Treatment
  • Pharmacy and Medication Assistance
  • Housing Assistance

FHC-Phoenix is a leader in serving the homeless.  In 2010, FHC-Phoenix received the Louisville Coalition for the Homeless’ “Outstanding Service Provider” award for its service to homeless persons in Louisville.  In 2013 FHC-Phoenix with the University of Louisville Hospital, received the Excellence Award for Housing by the Homeless & Housing Coalition of Kentucky (HHCK).  Mental Health America of Kentucky awarded FHC-Phoenix the Nick Baumgartner Award in the Fall of 2013 for its work in Trauma-Informed Care.  Housing First, a FHC-Phoenix program, has been recognized as a model project on Substance Abuse and Mental Health Services Administration’s (SAMHSA) website and was featured in the “National Council” magazine, published by the National Council for Community Behavioral Healthcare.
