CoC NOFO Information 2025

Continuum of Care Funding Opportunities

Each year, the U.S. Department of Housing and Urban Development (HUD) awards funding to Continuums of Care (CoC) through a competitive application process.  Each CoC must designate a “collaborative applicant” to submit applications to HUD on behalf of the entire CoC. 

The Coalition for the Homeless as the collaborative applicant for the Louisville/Jefferson County CoC, is responsible for coordinating local competitions and submitting a Consolidated Application (per funding opportunity) to HUD on behalf of the CoC. Individual projects requesting funding, both new and renewal, submit information directly to the Coalition for the Homeless.

A Consolidated Application consists of three parts:

  • a CoC Application completed by the Coalition for the Homeless responding to questions relating to system-wide initiatives and performance;
  • individual project applications;
  • and a “priority listing” of projects in rank order based on selection criteria set by the Louisville/Jefferson County CoC Board of Directors.

The FY25 Continuum of Care Funding Competition was opened by HUD on November 13, 2025. Additional information, including the full Notice of Funding Opportunity, is available here.

As of Tuesday, November 25, 2025 the Louisville/Jefferson County Continuum of Care officially began accepting applications for the 2025 CoC NOFO.

2025 Continuum of Care Annual Funding Competition Information

Applicants applying under the 2025 CoC NOFO must submit completed project applications, including for renewal projects and new projects, via HUD’s online application platform known as e-snaps.

Submitted applications will go to the Coalition for the Homeless for review and scoring and can be returned for additional edits. If selected through the local competition, the Coalition for the Homeless will submit your final application on your behalf to HUD by HUD’s final deadline of January 14, 2026.

Scoring Documents

The Continuum of Care Board of Directors approved the following scoring documents on November 25, 2025 for use in the FY25 Continuum of Care Funding Competition.

Checklist of Scoring Materials

**Note: Due to the temporary withdrawal of the FY25 CoC NOFO on December 8, 2025 updated local competition deadlines have been provided below. An eSnaps deadline will be provided as soon as possible.**

  • eSnaps Project Application*
  • Project Scoring Rubric to be completed by all agencies (see scoring guide for specifics) to CFH
  • Submission of Supportive Services Agreement to CFH
  • For VSPs Only: Submission of data source comparable to HMIS APR for the period of 07/01/2024 to 6/30/2025 to CFH
  • For New TH Projects Only: Submission of Letters to Commitment if a new project is leveraging housing or healthcare resources to CFH

  • Supplemental Budget Form (New Projects Only)
    • If you are using in-kind match, match sure to submit your fully executed match MOUs as well.

*If eSnaps project applications are not available in time for applicants to reasonably complete them, agencies will be provided a supplemental budget form to complete.

Individual Scoring Documents For Review/Completion

The following technical corrections were made to the above scoring documents on December 11, 2025

  • New Project + Projects That Have Not Completed One Operating Year Scoring Rubric
    • Clarifying that if your project is currently funded, but has not yet completed an operating year, you DO NOT have to complete the threshold Criteria Section.
    • Clarifying that the Bonus Points are only available to PSH projects applying as PSH.
  • PH+TH (no new projects) Scoring Rubric
    • Clarifying that the Bonus Points are only available to PSH projects applying as PSH.
